RECIST 1.1 Calculator
Track target lesions across scans and get the RECIST 1.1 response at each time point: sum of diameters, change from baseline and nadir, and the overall response from target, non-target and new lesions.
Target lesions
Up to 5 target lesions, at most 2 per organ. Measure the longest diameter for non-nodal lesions and the short axis for lymph nodes, in mm. Enter 0 if a lesion has disappeared, or 5 if it is still present but too small to measure. Leave a cell blank if the lesion was not assessed.

Report summary
Plain text for a report or trial note, based on the latest time point; read it over before pasting. The CSV has one row per scan with every lesion measurement, the sums, changes and responses. Scan dates are optional but are carried into both.
Enter baseline and follow-up measurements to build the summary.
Response by time point
| Time point | Sum (mm) | vs baseline | vs nadir | Target | Non-target | New lesions | Overall |
|---|
* Not all target lesions were measured at that time point.
Best overall response, confirmation of CR/PR (at least 4 weeks later in single-arm trials) and minimum SD duration depend on the protocol and are not computed here.
RECIST 1.1 rules used
- Measurable target lesions: non-nodal lesions at least 10 mm in longest diameter on CT (slices of 5 mm or less); lymph nodes at least 15 mm in short axis. Nodes of 10 to under 15 mm are pathological but non-target.
- Sum of diameters: longest diameter for non-nodal lesions plus short axis for nodes.
- CR: all non-nodal targets gone and every target node under 10 mm short axis, so the sum need not be 0. Nodes under 10 mm are normal, so a rising residual node sum alone is not PD.
- PR: at least a 30% decrease in the sum compared with the baseline sum.
- PD: at least a 20% increase over the smallest sum on study (the nadir, including baseline) and an absolute increase of at least 5 mm. After an overall CR, a target lesion that reappears is generally PD (a lymph node once it is 10 mm or more); after PR or SD, reappearance alone is not PD.
- SD: neither PR nor PD.
- Missing lesions: if a target lesion is not assessed, the time point is NE unless the measured lesions alone already meet PD. Incomplete time points are not used as the nadir.
- Overall response: any new lesion or unequivocal non-target progression is PD. Target CR with non-target non-CR/non-PD or not evaluated is PR. Target PR or SD stays PR or SD when non-target disease is non-PD or not all evaluated.
- Equivocal new lesions are not PD. If a later scan confirms one, PD is dated to the scan where it first appeared.
